  For å kunne vurdere om kvalitetskravet til helsehjelp er innfridd, må hjelpen være dokumentert. Helsehjelp som ikke er dokumentert, er juridisk sett ikke utført. Sykepleiere har et selvstendig ansvar for å dokumentere sitt arbeid i pasientjournalen. De juridiske kravene til journalføring samsvarer med trinnene i sykepleieprosessen. Innføring av elektroniske pasientjournaler forutsetter integrert terminologi og standardisert språk og er krevende for mange sykepleiere. Opplæring i bruk av slike journaler bør inn i sykepleierutdanningen.

  Objective: To identify the effectiveness of interventions to prevent corneal injury in critically ill, sedated, and mechanically ventilated patients. Research methodology: A systematic review of intervention studies was conducted in the following electronic databases: Cumulative Index to Nursing and Allied Health Literature, Cochrane Central Register of Controlled Trials, Embase, Latin American and Caribbean Literature in Health Sciences, LIVIVO, PubMed, Scopus and Web of Science, and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Study selection and data extraction were performed by two independent reviewers. Quality assessment of the randomized and non-randomized studies was performed using the Risk of Bias (RoB 2.0) and ROBINS-I Cochrane tools, respectively, and the Newcastle-Ottawa Scale for cohort studies. The certainty of the evidence was assessed according to the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system. Results: 15 studies were included. Meta-analysis showed that the risk of corneal injury in the lubricants group was 66% lower (RR = 0.34; 95 %CI: 0.13–0.92) than in the eye-taping group. The risk of corneal injury in the polyethylene chamber was 68% lower than in the eye ointment group (RR = 0.32; 95 %CI 0.07–1.44). The risk of bias was low in most of the studies included and the certainty of the evidence was evaluated. Conclusions: The most effective interventions to prevent corneal injury in critically ill sedated mechanically ventilated, who have compromised blinking and eyelid closing mechanisms, are ocular lubrication, preferably gel or ointment, and protection of the corneas with a polyethylene chamber. Implications for clinical practice: Critically ill, sedated, and mechanically ventilated patients who have compromised blinking and eyelid closing mechanisms must receive interventions to prevent corneal injury. Ocular lubrication, preferably gel or ointment, and protection of the corneas with a polyethylene chamber were the most effective interventions to prevent corneal injury in critically ill, sedated, and mechanically ventilated patients. A polyethylene chamber must be made commercially available for critically ill, sedated, and mechanically ventilated patients.

  Abstract. The paper presents a usability evaluation of the Graphical User Interface(GUI) of an Electronic Health Record System (EHR). The topic of interest was to explore the system’s usability in the context of nursing process documentation. A cognitive walk through approach was used. The data were analyzed with content analysis and the results show that challenges identified were related to navigating and finding information in the system. Even though there were problems in progressing from one phase to another in nursing process documentation, the system represented some types of predictability and consistency in the functions. Education, training and support are needed to be able to use the EHR for nursing documentation. Mandating standardization regarding format, content and terminology to improve the EHR systems functionality regarding facilitate nursing process documentation is recommended.

Aim: The aim of this study was to illuminate and interpret how good nursing care for older nursing home clients may be narrated by registered nurses in a Nordic context. Methods: Nine expert nurses from nursing homes in Denmark, Finland, Norway and Sweden narrated experiences of good nursing care for older people that were tape recorded, transcribed verbatim and analysed by means of qualitative content analysis. Results: Communication, creativity and community were the themes that constituted good nursing care for older people in the studied context. It was interpreted as a process of reciprocity between caregivers, clients and significant others, characterised by transcendence for the individuals that took part in the process. Discussion/Conclusion: Good nursing care in nursing homes demands mature individuals who are conscious and skilled professionals, able to create reciprocity in the care context. Such professional nurses could also serve as radiant role models to other registered nurses who are hesitant in entering nursing care of older people.   Policies and strategies to achieve welfare in the Norwegian society demand a well-developed and complex organization of public services. Statutory demands to deliver quality in public services are following the growth of the welfare state. In the purpose of learning from failure, it is required by law to report adverse events in the health services [1]. To support the increasingly complex public management that the growing welfare state requires, there is confidence in how information and communication technology (ICT) can support the processes. ICT systems are developed and implemented to facilitate processes and demands in public services, but display varying success. Implementation success depends on several factors[2]. System quality in terms of usability is one critical factor for success. The aim of this study was to evaluate the graphical user interface (GUI) of an ICT system module for reporting adverse events, developed for quality management in the municipality healthcare services in Norway.

  Hensikten med prosjektet er å fremskaffe ny innsikt i forbindelse med implementering av elektronisk pasientjournal (EPJ) i kommunehelsetjenesten og å bevisstgjøre på faglig, etisk og juridiske krav. Det holdes kurs i kommuner og gjøres forskning på implementering av EPJ i kommunehelsetjenesten. Utfordring i forbindelse med tilrettelegging for opplæring. Det er en utfordring i forbindelse med bruk av ny teknologi. Kommunene trenger samarbeidspartnere. Vi vil utvikle en faglig forsvarlig dokumentasjon i praksis ved hjelp av EPJ med integrering av klassifikasjonssystemet SABAKLASS og registreringssystemet IPLOS.
